Ryota Sakamoto is an unemployed twenty-two-year-old living with his mother who spends his days dominating the combat video game Btooom! Without warning, he wakes up stranded on a tropical island with no memory of how he arrived. He soon discovers a strange crystal embedded in his left hand and a bag filled with explosive weapons known as BIM.
After a stranger aggressively attacks him with a bomb, Ryota realizes he has been thrown into a lethal, real-life version of the game he mastered online. With no reset buttons or second chances, he must eliminate other unwilling participants to secure his escape. While navigating the treacherous jungle and trying to stay alive, Ryota encounters Himiko, another skilled player who happens to be his in-game wife.
Together, they form an alliance to endure the extreme psychological and physical horrors of the island. Meanwhile, back in the civilized world, an investigation led by Iida, government agents like Matthew Percier, and Ryota's family members works to uncover the sinister forces pulling the strings behind the deadly tournament.
