In the city of Mitakihara, a group of girls including Madoka Kaname, Sayaka Miki, Mami Tomoe, Kyoko Sakura, and newcomer Homura Akemi find themselves fighting against entities called Nightmares. However, cracks soon form in this seemingly peaceful routine when Homura and Kyoko realize they are actually trapped inside a magical witch's labyrinth. It becomes clear that their memories have been manipulated.
Homura eventually discovers that she is the one responsible for the false environment. After previous reality-altering events involving Madoka, Homura had fallen into deep despair and transformed into a witch, trapping herself inside an isolated Soul Gem. The alien creature Kyubey and its species, the Incubators, orchestrated this isolation in hopes of capturing Madoka and regaining control over how magical girls generate energy.
As Kyubey tries to seize control of Madoka, Homura fully turns into the witch Homulilly, refusing to let the aliens capture her friend. Her allies, including Sayaka and Nagisa Momoe who now serve as guardians of the cosmic order, arrive to help break the barrier imprisoning her.
