Long ago in 1964, Ed and Lorraine Warren investigated an antique mirror haunting where Lorraine experienced a terrifying vision and temporarily lost her newborn baby, Judy, before prayer miraculously revived her. Twenty-two years later, in 1986, the Smurl family moves to West Pittston, Pennsylvania, sharing their new home with Jack, Janet, grandparents Mary and John, four daughters, and their dog. John gifts Heather an antique mirror for her confirmation, sparking a series of increasingly violent paranormal disturbances involving multiple ghostly apparitions.
Meanwhile, Ed struggles with a worsening heart condition, leading the Warrens to officially retire from active investigations and focus on lecturing. However, Judy experiences growing psychic sensitivity and panic episodes. After Father Gordon tragically dies following a demonic attack at the diocese while looking into the Smurl haunting, Judy discovers the connection and secretly travels to Pennsylvania. Ed, Lorraine, and Tony follow her to the Smurl residence, where the Warrens realize the antique mirror is actually the exact same demon from their very first terrifying case decades earlier, now specifically hunting Judy.
